Hospice Care

Hospice is a special kind of care designed for individuals with a Terminal Diagnosis. Hospice is more than the last few days of someones life. Hospice emphasizes palliative rather than curative treatment; quality rather than quantity of life. The patient and their family are both included in the plan of care; emotional, spiritual and practical support is given based on the patient’s wishes and family’s needs.

Above & Beyond Hospice Care provides the highest quality, compassionate end-of-life-care to individuals with a life-limiting illness, along with their family and caregivers. Services are provided to individuals in their home, assisted or independent living environment and long care facility within a 50 mile radius of Monticello.

Every Hospice patient receives an Interdisciplinary Team of professionals. This includes an attending physician, hospice medical director, registered nurse, hospice aides, social worker, chaplain, and volunteers.
